Legend & History of the Neelkanth Mahadev Rishikesh:

This poison, which was emitted during the Samudra Manthan (ocean churning), turned his throat blue. As a result, Shiva is also known as Neelkanth, which translates as “The Blue-Throated One.”

According to the Shruti-Smriti Puran, Devas and Asuras churning the ocean in order to acquire Amruta. One of the 14 ratnas recovered was the “Kalkut” poison, capable of destroying the entire globe. Lord Shiva ate this, turning his throat blue. Lord Shiva meditated for 60 thousand years here at the junction of the Pankaja and Madhumati rivers beneath the Panchpani tree, where the modern Garbha Gruh of the temple is built, to alleviate the effects of Kalkut poison.Lord Shiva built the existing “throat” shaped shivling beneath the Panchpani tree after his penance.
